Job Description Job Scope: Reviews all medical records to assign coding classification, as per legal and insurance rules and requirements. Utilizes patient databases and IT System to conduct analysis, research and develop internal reports, as per administrative requirements. Supports hospital initiatives by conducting training and implementation of coding practices across clinical and non-clinical areas. Job Responsibilities: Reviews medical record to confirm patient’s diagnosis and procedure; and consult with physicians for clarification prior to code assignment when required. Assigns CCHI, ICPM & ICD-10 codes in Patient Database and SAP system to ensure accurate coding of diagnoses and procedures. Creates monthly report containing all discharge indicators required by Ministry of Health to assure timely submission. Collaborate with Quality Department to report Hospital KPIs related to readmissions, transfer, DAMA, M&M cases, etc. Partnering with discharge clerks to resolve discrepancies related to admission and discharge patient information. Maintains confidentiality of patient’s file, guides against misuse of information contained therein, and report incidences to Quality Department for investigation. Assists HIM Statistical Unit by submitting needed data for internal and external reports generation. Supports Hospital-wide initiatives by providing coding training to clinical and non-clinical staff. Evaluates coding practices by conducting ongoing quantitative and qualitative audits. Stays abreast of coding & medical terminology updates, Accreditation and MOH coding requirements via research activities and professional trainings. Job Requirements: Education and Experience Bachelor University Degree in Nursing. Additional certification in coding is required. 1 to 3 years of experience as a Medical Coder. Job-Specific/Technical Skills Excellent knowledge of related accreditation and certification requirements, and knowledge of medical terminology. Excellent computer skills Excellent organization and problem-solving skills. Strong verbal and written communication skills. Strong attention to detail and good analytical skills. Ability to quickly learn IT solution changes/ enhancements. Following established policy and procedure for the handling and safeguarding of confidential files and records.
